The International Breast Cancer Study Group (IBCSG) are really looking out for our ladies. They
decided to do the Tamoxifen and Exemestane Trial (TEXT) and the Suppression of Ovarian Fun-
ction Trial (SOFT). Those are phase 3 studies. The researchers did an examination of the adjuv-
ant, hormone therapy options.
Those particular ones are for premenopausal ladies with estrogen-receptor-positive breast cancer.
The IBCSG made sure that the trials compared the years of ovarian function suppression (OFS)
combined with tamoxifen or exemestane. Tamoxifen-monotherapy was used in the Suppression
of Ovarian Function Trial.
Ovarian function suppression was accomplished with five years of triptorelin, oophorectomy, or
ovarian irradiation. The results from the trial showed SOFT (n=3,066) & TEXT (n=2,672) regard-
ing a median follow-up of 5.7 years.
The survival rate of the ladies who were given ovarian function suppression with exemestane was
91.1 percent. The ladies who got ovarian function suppression with tamoxifen topped out at 87.3
percent. Now, you know why OFS with exemestane is highly recommended.
That combination does cause some patients to lose bones. If GnRH agonist is utilized, the estr-
adiol will have to be watched closely. The IBCSG did the Prevention of Early Menopause Study
(POEMS) that helped prememopausal ladies with the hormone-receptor-negative breast cancer
in its early stages.
Then they were sent for chemotherapy with goserelin (n=126) and without goserelin (n=131). The
ladies who were given 3.6 milligrams of goserelin got well. They got that before chemotherapy, ev-
ery four weeks while going through chemotherapy, and within the two weeks of the end of chemo-
therapy. The rate of ovarian failed two years in and they did call it amenorrhea six months before.
The FSH level in the postmenopausal range was actually lower in the ladies who were given gose-
relin (8% against 22%). Goserelin had a pertinent connection with a high rate of completed pregn-
ancies in ladies who went through those (18 babies against 12 ones). We should appreciate every-
thing that is being done to help our ladies.
